Its very rare to fail it happened to me only once
considering I made x2 lvl 36 gen 1 fairies
Str pet lvl 41 , Agi pet lvl 41 and con pet lvl 41 (2nd gen)
leveled them all from scratch and it happened only once on my con pet

From my experience, highest fail rate is around lv13-20, lv20+ can still fail but it should be lower.

I played 10 years of top! And here in pko more than a year and a half, the first time I fail, so I found it strange, I thought only the great gruit failed.
