how does Effectiveness works? And how can I make an item get to 250% effectiveness?

So...I was always looking for someone that could tell me how to make an item have increased effectiveness...
Can someone make a Effectiveness tutorial or something like that for me and other newbies to understand how can we do it and how does it work please?

Each time you successfully upgrade your Equipment (using a Strengthening Crystal and Strengthening Scroll) it increases by 2%, and it can go up to 110%.

Yes, you can only get it to 110%, which requires 5 successful strengthenings.

Apparel Upgrade at Shaitan Blacksmith, cost is from 1.2m-2.2m, depending on the Effectiveness your Equipment has.
